This is a very common top-level exception. It usually occurs if a tool within the action returns a high return code or encounters a problem. A tool which returned a high return code most probably also returned a reason message. The reason message should be visible under "Reason" in the error dialog or can be found in the error details.

Contact the model administrator if the reason given by the executed tool is not clear or if there is no reason message at all.
