This is a very common top-level exception. It occurs if any modeled tool returns a return code which is higher than the modeled "Max_RC", which is an attribute of the "Application Options" ("4" by default). A tool which returned a high return code most probably also returned a reason message. The reason message should be visible under "Reason" in the error dialog or can be found in the error details.
